Illinois Healthcare Nursing Home Resident Background Check is $45

Your employer will provide you with a valid live scan request form from the Illinois Department of Public Health for Nursing Homes (IDPH). This form must be completed by your employer.

Bring the completed IDPH livescan request form and a valid government-issued picture ID (such as a Drivers License, State ID or US passport).
A receipt noting the required Transaction Control Number (TCN) will be provided to the applicant at the completion of the fingerprint capture process.
Your results will be sent directly from the Illinois State Police to the Illinois Department of Public Health (IDPH) typically within 72 hours (based on the results of your background check).
The Illinois Department of Public Health will e-mail the results to the facility (your employer) and also update the IDPH Health Care Worker Registry.
