Identification Required

Bring a valid government-issued ID (Drivers License, State ID, Passport, Military ID). Only one form of ID is needed.

After you have been fingerprinted you can have a copy of your Illinois "Rap Sheet" mailed directly to you.

What if my Illinois Criminal History Record is Incorrect?

You can challenge any inaccuracies in your Illinois Criminal History Record by submitting a Record Challenge form. The Record Challenge form will be included with your results. Mail the completed form to the Illinois State Police for their review and response.
