Things to bring to your appointment in Joliet, IL

• Bring a valid government-issued ID (Drivers License, State ID, Passport, Military ID). Only one form of ID is needed.
• Bring your completed contact information form

Background check results will go to the Illinois Department of Financial and Professional Regulation (IDFPR) typically within 72 hours.

Out of State Applicants Follow these steps:

• Review the fingerprinting checklist
• Complete the contact information form
• Provide 1 or 2 completed fingerprint cards in black ink or digital. We recommend sending 2 cards in case 1 card has imperfections. The fingerprint cards may be printed on regular office paper. Use a large envelope and write "do not bend" on the outside. NOTE: Do not bend the cards.
• Complete the Identity Verification Certifying Statement
• Provide a copy of your driver's license or State ID, or a copy of your U.S. or Foreign Passport. Please write your email and phone number "CLEARLY" on the copy of your ID/Passport
• Enclose a U.S. money order in the amount of $60.00 USD.

After your payment has been received and completion of the fingerprint card scan process you will receive an email receipt with your transaction contol number (TCN).

Your fingerprint based background check response will automatically be sent from the Illinois State Police to IDPFR typically within 72 hours.
